It was an enjoyable and productive day – but as a GP Commissioner, my major interest – at any rate initially – lies in preventing patients such as Fred, with multiple co-morbidities from being inadvertently harmed by actions taken on one pathway impacting on another e.g. NSAIs for rheumatoid arthritis or pain control in a patient already on an ACE Inhibitor **simply because the care providers on one or other – or both – of the pathways did not know what other Pathways were involved nor what medication Fred was taking**.
We seem to have missed out the Care Coordinator in the Etherpad notes – but there were some technical problems: glad the problem seems fixed.
